Here is where we would have an introductory letter from a senior manager within your organisation explaining the survey purpose. For example:

I am pleased to invite you to complete the ABC Employee Opinion Survey. This survey has been designed to give information on how you feel about working at ABC, the relationship ABC has with you and how well your training and career needs are being met.

This year we have asked an external company, People Insight, to conduct the survey on our behalf and your answers from the survey will go directly to them.

The questionnaire should be completed no later than DATE. Once the results have been analysed the feedback will be communicated and issues identified will be discussed and a plan of action agreed.
